Possibly there is a surge on power-up that creates the condition. If you have access to a scope multimeter, scope the mainline power to the machine on power-up.

If a fault is still active following a Fault Reset service, the fault is again posted to the display and written to the fault log. The drive maintains a log of the last 25 faults with time stamps and stores the fault log in persistent memory. However, the fault log cannot be cleared on the drive.
